Individualized Care

Supervised by our Medical Director and Director of Nursing, our skilled nurses are available everyday during open hours. This includes private sessions with participants to allow any questions or concerns they may want to voice, checkingr vitals to ensure healthy goals are being met, and educating participants, caregivers, and staff. Our nurses administer doctor prescribed and over the counter medications. Personalized Plan of Treatments are created for each participant with assistance from their Individualized Disciplinary Group. The Plan of Treatment revolves around the participants health needs and allows each participant to work on goals that encourage the highest quality of life possible.

We have showers, tubs, supportive devices, and durable medical equipment for those that may need them. Our laundry skills room is equipped with a washer and dryer for use with assistance and training, if needed. A few times a week we provide pedicures and manicures and various hygienic therapies. A nutritious breakfast, lunch and snack are provided every day. Exercise programs which includes visits to a local gym with a personal trainer, dancing, walking, or using the on site exercise equipment. All of our activities are designed to educate, enhance and improve our participants wellbeing.

Our participants are able to enjoy the various events in our wonderful community. We go out into the community at a minimum of once a week. During operating hours, we provide scheduled transportation to food banks, health fairs, craft events, mission stores, flea markets, grocery stores, and so much more. Visiting the Arboretum, watching shows at the planetarium, and walking around the Salanto Wildlife Refuge are just a few of the things we have been able to enjoy. 

Our participants are given the opportunity to socialize with various people from different walks of life on a daily basis. Some participants have developed very close relationships. We are able to assist the clients to connect with long distance family and friends by writing letters, setting up emails or by calling, if appropriate. We have guest speakers who entertain and educate our participants. This interaction provides a beneficial social setting not many be able to experience elsewhere.

We have fall festivals, ice cream socials, circus events, and theme weeks all through out the year. Our participants are able to choose what theme they want to have and the staff create it for them while promoting the participants personal goals.

 

 

Everyone has the ability to learn something new everyday. We provide the atmosphere to encourage learning new skills and improve on already learned ones. We offer sewing (either by hand or sewing machines) lessons, computer and technology lessons, cooking and nutritional lessons, art and artistic expressions, music expression and musical instrument lessons, budgeting and money management skills, and much much more. We intergrate different approaches to reach out to everyone's specific learning style.  Each person is different so it takes different ways to reach everyone. We encourage the participants to work on their goals by educating and providing the opportunities to learn.

Each participant has a specialized group of professionals (usually participants doctors, counselors, and etc) caregivers, and family members that work closely with the participant to help create and maintain a plan of care that is the most beneficial to the participant. This group is called the Individualized Disciplinary Group (IDG). This group meets once a month and when necessary. This group is actively interdependent, with an established means of ongoing communication among team members and with patients and families to ensure that various aspects of patients' health care needs are integrated and addressed.
