We are members of Berea Chamber of Commerce, KAAD (Kentucky Association of Adult Day centers), NADSA (National Adult Day Service Association) and NAMI (National Alliance on Mental Illness).
We are working with Veteran's Care Coordination to provide services to our valuable Veterans and their spouses.
We partnered up with AFA (Alzheimer's Foundation of America) to offer
FREE Alzheimer's/Memory Screening to the community, every 2nd Tuesday of the Month.
We are certified by the OIG (Office of Inspector General) and licensed by the State of Kentucky.
IDG (Interdisciplinary Group) meetings are once a month and as needed.
The interdisciplinary group consists of practitioners from different professions who share a common patient population and common patient care goals and have responsibility for complementary tasks. The team is actively interdependent, with an established means of ongoing communication among team members and with patients and families to ensure that various aspects of patients' health care needs are integrated and addressed. The participant takes an active role in his or her care.
So we can best serve the individual to meet the needs and choices he or she voices.
Dr. Nawar Soda, is a graduate of Damascus University and a Board Certified Medical Doctor in Family Practice and operates the Urgent Care Center's of Kentucky, with two locations to serve you in Berea, as well as Rural Family Practice and Medical Laser.
